Utility Forms:

• Utility Consultant Approval Form : This form is to be completed by the Utility's Consultant Engineer for approval to perform Utility Engineering on a Highway Project.

• Utility Permit: This is to be completed by a Utility Company for approval to Occupy Highway Right-of-Way.

• Utility Project Inquiry Data Sheet: This is to be completed by all utility Companies Officially Notified of a Highway Grading Project.

• Preliminary Utility Information Form DOT-248: This form is to be completed by the Utility Companies to obtain data on existing facilities. This data will be used in project scoping and design. The date field is automatic and does not need to be filled in.
