You can check the version by starting Acrobat Reader, selecting "Help" from the menu bar and then selecting "About Acrobat Reader" -- your version of Acrobat Reader will be shown near the top of the opened window. If you do not have Acrobat Reader installed, you can download the free Acrobat Reader plug-in. If you have trouble accessing the new PDF E-forms that can be filled out online, printed, and mailed in, try upgrading your Internet Explorer or Netscape browser to the most current version.
