In order to complete an online PDF form you need to have Acrobat Reader version 5.0.5
or higher installed on your computer. You can download the free
Acrobat Reader plug-in.
If you have trouble accessing the new PDF E-forms that can be filled out online, printed,
and mailed in, try upgrading your Internet Explorer or Netscape browser to the most current
version. These forms will not work with the older versions of IE & Netscape.
